In Phase VII, Take Action, community assets are mobilized to achieve the targeted community outcomes identified in the community impact plans. Each priority issue will have its action plan(s) implemented by a team of individuals dedicated to seeing the plan(s) happen in their own community.

These implementation teams can use these plans as blueprints for addressing the priority issues in the community. They also can use the results of the process for leveraging additional resources, both locally and nationally.

Phase VII, Take Action, is a crucial phase in the community-building process. Each implementation team is responsible for ensuring that their community impact plan is implemented.
