Phase VIII Track Process, Progress and Impact

In the final phase, Phase VIII, Track Process, Progress and Impact, the process for conducting the effort is assessed.

The Implementation Teams ask:

  • Is the process working effectively?
  • Is the process staying on track, on budget and on schedule?
  • Does the process need to be changed? If so, how?

Periodic progress reports are completed to track activities and achievements and to make mid-course corrections.
Community partners on the Implementation Teams update the community impact plan to reflect emerging assets and issues in the community, and monitor the impact by comparing actual community outcomes with the targeted community outcomes and community vision set forth earlier.

Among the questions answered in Phase VIII are:

  • How has the community changed as a result of the actions taken?
  • How have the community indicators changed?
  • Based on this information, does the community impact plan need to be adjusted?
